Welcome to the UHS Alumni Survey! 

We are collecting information from UHS Alumni with the intent of building a strong social network for alumni and strengthening resources for current UHS students, including the Penguin Pathways event, college/career counseling services, and alumni feature articles for UHS official publications. 

Your privacy is important to us; your responses will remain as confidential as you choose. In the Privacy section of the questionnaire, you have options on whether or not you want to share some of your responses with different audiences. The options are as follows: 

  1. Coordinator of Penguin Pathways, a student/alumni program
  2. Other alumni, for networking purposes
  3. UHS college counselor(s), to potentially assist current UHS students
  4. UHS faculty sponsor of the alumni feature section in the student online magazine, UHS Perspective, and the principal's newsletter (for a student/parent audience) entitled "The Weekly Penguin".

If you indicate "yes" here, the coordinator of the Penguin Pathways event will contact you to discuss participation. The 2017 event will be held at UHS on September 15, from 8:30 AM - 11:45 AM. The goal is to help students understand their education at UHS as a part of their larger growth as individuals, and help them make informed decisions about their futures. UHS alumni panelists will share their college/career stories with students during 3 sessions.
For networking purposes, are you willing to share your responses to questions in the networking section with alumni who are logged into the UHSFAA.org website?
Please indicate whether you are willing to share your name, email address, and survey answers to pertinent questions (college, networking, and support) with the UHS college counselor responsible for placing current students. No other contact information with be shared. .

He will share them with student journalists for potential publication per your instructions.
